Care Manager - Home Care

We are a rapidly growing home care organization in Chattanooga, seeking a compassionate, hands-on Care Manager to support clients and caregivers while helping to drive operational excellence.

What You’ll Do
  • Conduct client assessments and create individualized care plans.
  • Perform in-home visits, reassessments, and provide ongoing client support.
  • Supervise, train and coach caregivers to maintain quality and service standards.
  • Introduce caregivers to clients, review expectations, and ensure consistent care delivery.
  • Maintain accurate documentation and coordinate with community providers.
  • Conduct initial intake conversations with the prospective client or family member/ guardian to assess needs and determine service fit. 

What We’re Looking For
  • 2+ years of experience in home care, home health, or a related field.
  • Strong communication, organization, and follow-through skills.
  • Ability to supervise caregivers and advocate for clients.
  • Reliable transportation, valid driver’s license, and insurance.
  • Professionalism, compassion, and dedication to serving others.

Compensation: $45,000.00 - $55,000.00 per year
